Sport Physicals

The UI Family Care – Sigourney clinic will be offering sports physicals for the upcoming 2009-10 school year. They will be holding these clinics specifically for school sports physicals. The dates for these physicals will be Monday, July 13; Monday, August 3 from 4 until 7 p.m. If there is a need, more dates may be added. The students may either walk-in or schedule ahead of time by phoning the clinic at 641-622-3840.
These physicals will be offered at a special rate of $25. This is a
special rate and will be due at the time of the physical.
No physicals will be submitted to insurance.
The clinic will once again be able to donate a portion of the fee
for the physical back to the athletic department of the school that
the student attends. We are only allowed to offer this special rate
for a limited amount of time and only the physicals that are done
on these dates are eligible for the donation and the $25 price. Our
staff is volunteering their time to work at these clinics in order
to be able to make this event happen.
Please make sure that your athletes have forms available for the
physicals and that the parental and student portion is completed
before they arrive for their physical. The clinic will have limited
amount of forms available.
All students not accompanied by a parent must have a signed
permission slip in order to be seen.
