TO ALL 9TH GRADE PARENTS:
It is required by STATE OF IOWA law that all freshmen have a dental exam. A representative from Public Health and the I Smile Program will be here in the near future to evaluate our freshman class records. As of today, we are extremely short on returned dental forms. This is an audit that goes into the state and is published for public viewing.
This required form is available at ALL dental offices. If your child has had an exam, please have your dentist fax the REQUIRED form to Pekin Schools at: 641-661-2351 or 319-695-5130. If your child has not had an exam, please make an appointment as soon as possible.
